Becki Koldsbaek - As your Inside Sales/Customer Service Representative, Becki is available during business hours to assist with status, pricing, scenarios, and condition questions. Once your file has been submitted Becki and you will work hand in hand throughout the process. She will be in constant communication by phone or email as your file progresses. Forms
Submission Form. You may submit by completing the submission form and emailing it to Becki or me along with a 1003, 1008, and recent credit report. Alternatively you may fax the same documents to 503-635-6548. This will produce a fast track pre-approval in 24 hours. If you have other documents it is preferred you email or transbox the entire file so that we may begin the underwriting process.
Lock Form. To lock your loan please fill out the Lock form and fax it to 503-635-6548. You will receive a confirmation within 24 hours. Alternatively you may email your lock request to Becki or me.
Doc Request. Once all prior to docs are met you can submit your doc request. Please contact Becki Koldsbaek at 503-635-6504 to coordinate the doc drawing process.
Business Profile. As investor appetite in the market shifts we are required to present more evidence to support income on our stated transactions. The business profile allows you and the borrower to explain their company or position to a greater extent than a 1003 or 1008 can convey. This allows the underwriter the ability to present a more complete picture to the investor. I recommend you include a business profile with all self employed stated transactions.

This will allow you to check the guidelines for each product we offer. Alternatively the rate sheet offers matrices for each product on the third page.

Application
To become a First Horizon broker, please follow the steps below:
1. Download and complete the following documents and mail them to my attention at our Lake Oswego Office
2. Provide copies of the following documents:
- License
- Resumes of the principal officers of your company
Subsequent Branch Approvals.
Each branch of the Broker is not required to be individually approved or assigned a broker number to originate and submit loans to FHHLC. A branch may be assigned a separate and destinct broker number upon request once their corporate office has been approved.
Branch Approval Procedures:
* Each branch location must be identified and the following information provided:
- Branch Application Form must be completed and signed by a Branch Manager certifying that the branch is authorized to submit loans to FHHLC under the corporate agreement and that the branch is licensed according to state requirements.
- Copy of Mortgage Broker License for any state in which the branch will be originating loans for delivery to First Horizon.
